†The Accu-Chek Circle+ Program is a patient assistance program designed to provide valuable assistance by offering support tools to the patient and reducing some of the costs related to the illness. The patient may receive up to a maximum of $7 in patient assistance per box of Accu-Chek strips/cassettes prescribed and dispensed. Applicable on boxes of 100 or 102 Accu-Chek strips and boxes of 100 Accu-Chek Mobile tests (2 cassettes of 50) only. The patient assistance amount varies and is based on the patient plan coverage and provincial pricing for Accu-Chek strips/cassettes. For this reason, it may be less than the $7 maximum. To take advantage of the other Circle+ benefits, the patient has to register with Roche Diagnostics within three months of the first usage (business reply card, www.accu-chek.ca/circleplus or 1-800-363-7949). The Accu-Chek Circle +Program is subject to change or cancellation without notice. Valid for Canadian residents only in participating stores.
